The RC2 will work in the required location augmenting the existing staffing complement and acting as back up in the event of a reduced team in either location.
Program Summary:
The 24/7 Home for Good Program is a residential program with staffing on-site 24/7 for individuals who require supportive and supervised living in a structured environment. This program focuses on individuals who are unable to achieve or maintain housing without intervention. It requires crisis intervention, facilitation, and assistance with ADLs such as groceries, cleaning, appointments, etc. The clients in this program are individually housed in self-contained units and the goal of the program is for those units to become their permanent homes. This is a 12-unit program, with two (2) crisis units, which are used for short-term crisis solutions for individuals who have alternate residences, the crisis units are not used as a homelessness solution.
The Back-to-Home Program provides client-centered, community-driven solutions to reduce Alternate Levels of Care (“ALC”) waitlist pressures. Under this program, clients receive both support services and housing assistance. However, as individuals’ needs change, supports may be adjusted as appropriate. Clients work together with the Residential staff, Case Managers, Addictions Counsellors and other wrap around supports.
