Hours of Work:      9:00 am to 5:00 pm Monday to Friday (35 hours per week)

Salary Range:        $69,032.60 to $80,735.20 per year plus a comprehensive benefit package

The John M. Parrott Centre is a fully accredited state-of-the-art, 168 bed long-term care home owned and operated by the County of Lennox and Addington in the Town of Greater Napanee.  The John M. Parrott Centre offers free parking, a safe work environment and upgraded technology.  We are currently seeking a highly motivated and energetic Resident & Family Liaison to join the community of The John M. Parrott Centre.

 QUALIFICATIONS/EXPERIENCE:

·          Diploma in Social Service Worker or equivalent, and education related to Gerontology, Adult Education

·          Excellent planning and organizational abilities

·          Must possess strong written and verbal communication skills with the ability to facilitate and present clearly and effectively to groups

·          Effective team-building skills, including fostering a positive and cooperative work environment

·          Knowledge of Person-Centered inclusive approaches to care and services

·          Client services and public relations skills

·          Computer software skills, including Word, PCC, case management and data tracking

·          Knowledge of community supports to assist families through the journey of their loved ones (i.e. Alzheimer’s Society, Seniors Financial Supports etc.)

·          Knowledge of resident-focused care (i.e. General Persuasive Approaches (GPA), Dementiability)

·          Satisfactory attendance record, subject to the requirements of the Ontario Human Rights Code 

DESCRIPTION/RESPONSIBILITIES OF THE POSITION:

·        Active participation in intradepartmental committees, including CQI, PAC, Collaborative rounds, Wellness Reviews and Ethics Team (data tracking/analysis and presentations expected)

·        Provide encouragement, emotional support and transition support to residents, staff, families, persons of importance to residents and volunteers

·        Develop and implement programs and policies for Support Services that meet the Fixing Long Term Care Act and Regulations Compliance Standards

·        Responsible for providing residents with in-house access to community resources (to assist with Finances, taxes, involuntary separation, accessing financial supports, passport funding navigation, mental health supports and other relevant external partners)

·        Participate/advocate/support during Wellness Reviews and Move-in Day to assist residents and families

·        Share relevant information that will support the team in the creation of person-centred care and service delivery 

REGULATED SAFETY ACTIVITIES:

·        Must adhere to all service area / The John M. Parrott Centre policies and procedures as outlined in WHMIS, Infection Control, Fire & Safety, other manuals and legislation

·        Responsible for adhering to all Occupational Health and Safety procedures of the Act and the Home
