Volunteer and Programs Coordinator

The Volunteer & Programs Coordinator plays a central role in ensuring compassionate support for individuals, caregivers, and families connected with Hospice Prince Edward. This position is responsible for all aspects of the volunteer program — including recruitment, onboarding, scheduling, and engagement — and will oversee two key service streams: Grief & Bereavement Support programs and In-Home Volunteer Support.  He/She will  work closely with clients and their families in a variety of settings, assisting individuals and families to optimize their End-of-Life journey through our volunteer support services. 

The ideal candidate is organized, warm, an excellent communicator, and passionate about supporting volunteers and community members during some of life’s most vulnerable moments.

Responsibilities and Duties:

Volunteers

  • Lead recruitment, screening, interviews, reference checks, and volunteer intake

  • Ensure that ALL volunteers (with the exception of event volunteers) are enrolled and complete the 30 hour HPCO training program, with ongoing updates as available or required

  • Maintain volunteer records, documentation, and compliance requirements
    Tracks volunteer hours for report submission

  • Arranges for and hosts volunteer meetings as required and communicates regularly with volunteers working in the community.

  • Plans for regular volunteer recognition and recognizes individuals for significant achievements.

  • Consults with the ED to problem solve a volunteer and/or volunteer-client relationship issues.

Clients

  • Receive requests from community agencies, PEFHT or other care providers regarding clients who need either G&B support or In Home visits.

  • Meet with the client and/or family to determine needs and ensures that a care plan is in place  with an appropriate volunteer.

  • Communicate regularly with the volunteer and client to monitor progress and concerns, ensuring a supportive and goal-focused relationship.

 In Home Volunteers

  • Promote the InHome Volunteer Program within the community.  Ensure that local doctors, nurse practitioners and other health partners are aware of the program.

  • Oversee referrals and client intake process.

  • Match volunteers with clients based on needs and availability.

  • Monitor volunteer visits and follow up with clients/families as needed.

Grief & Bereavement

  • Promote the Grief & Bereavement  Program within the community.  Ensure that local doctors, nurse practitioners and other health & community partners are aware of the program.

  • Support the Volunteer Leadership Team in organizing and conducting support group sessions & One-on-One companioning sessions.

  • Coordinate & support group logistics – advertising, registration, scheduling, communication.

  • Support facilitation (may co-facilitate).

  • Maintain resource materials and brochures relating to these programs.

Information Management

  • Maintains all volunteer files, both paper and electronic on the case management software system.

  • Create, maintain, update and discharge confidential client files in the case management software system.

  • Maintain monthly contact and reporting for client status and case management.

  • Track statistical data and submit monthly reports regarding volunteer activities and levels of participation in programs for the Health Services Provider/Funder, the Executive Director, and the Board of Directors.

Program  Management

  • Collaborates with the ED to develop plans to enhance both programs based on community needs.

  • Contributes to conducting environmental scans of the community to identify and understand emerging local issues and opportunities for volunteer involvement e.g., issues of diversity.

  • Establishes and maintains community relationships for referrals and program development.

  • Represents Hospice Prince Edward for Community Outreach presentations and meetings as required.


Qualifications and Experience:

  • A degree or diploma in social services, or equivalent, or a proven combination of relevant education and experience.
  • Knowledge and experience in community-based end-of-life care for clients and family.
  • An understanding of and experience with the grieving journey.
  • Leadership experience with group facilitation and training for grief support in a diverse population.
  • Proficient in Google and Microsoft Office.
  • Knowledge of hospice services, health care, and relevant community resources in Prince Edward County.
  • Demonstrated ability to work as a collaborative and respectful team member.
  • Demonstrated excellence in communication, interpersonal, organizational, and time management skills.
  • Empathetic, patient, and approachable.
