H.J. McFarland Memorial Home in idyllic Prince Edward County is accepting applications from Registered Nurses for the position of RN: Care Coordinator. Reporting to the Director of Care, the Care Coordinator is responsible for providing the highest level of nursing care in accordance with the College of Nurses of Ontario and RNAO Best Practice Guidelines, physician orders and administrative policies.
Working Monday to Friday, the Care Coordinator prepares, implements, reviews and evaluates all resident care plans and monitors summaries, residents’ health, notes changes in conditions, and assesses the need for doctor referrals. The Coordinator provides advice on resident care needs in various settings, leads the Falls and Restraints program, participates in the medication management program, and acts as backup for the RAI Coordinator and the IPAC Lead when the incumbents are away.
The Care Coordinator is accountable for working collaboratively with the other nursing team members to provide quality resident care, ensuring resident safety by following relevant acts,
regulations, professional bodies, policies, and directives from the Ministry of Health, Long-Term Care and CARF Accreditation, and for supporting the achievement of the goals and objectives as set out in the Mission, Vision and Values of the Home.
