Youth Habilitation Quinte Inc.’s housing program, Next Step Housing Connection, is seeking a Homelessness Prevention Case Manager to provide case management services to individuals experiencing chronic homelessness or at imminent risk of homelessness in Hastings County. This is a permanent position, 35 hours per week, commencing Jan 5th, 2026. The role includes vacation time, sick leave, benefits and pension plan.
Who We Are:
Next Step Housing Connection provides free housing support services to low-income individuals (aged 16+) and families experiencing or at risk of homelessness in Hastings, Prince Edward, and Lennox & Addington Counties.
Who You Are:
You are someone who thrives on supporting individuals facing complex life challenges. You’re empathetic, non-judgmental, and skilled at building trust with people who may be experiencing mental health concerns, addictions, trauma, and instability. You can balance compassion with strong boundary-setting and are comfortable working independently in the community. You are self-directed, well-organized, and confident in your ability to problem-solve in fast-moving situations. You communicate clearly, maintain accurate documentation, and are committed to helping clients achieve long-term housing stability.
Your Role:
As a Homelessness Prevention Case Manager, you will provide intensive, client-centered case management services to individuals experiencing chronic homelessness or at imminent risk. You will support a small, targeted caseload identified through the By-Name List and Coordinated Access, conducting housing assessments, developing individualized case plans, and delivering ongoing follow-up and homelessness-prevention supports. Your work will include assisting clients with housing searches, accompanying them to viewings, helping with applications, coordinating moves, and supporting them in maintaining tenancies using a Housing First, low-barrier approach. You will also provide life-skills coaching such as budgeting, tenancy education, and housing-retention strategies, while educating clients about available community supports, benefits, and emergency resources.
A key component of this role is coordinating services with other community partners and service providers and liaising with Landlords. You will take a lead role in collaborating with local agencies, participating in case conferences, facilitating referrals, and ensuring clients receive integrated, wraparound supports. Strong relationship-building and interagency coordination will be essential to supporting client success.
Skills We’re Looking For:
· Experience administering housing assessments and developing case plans
· Strong crisis-intervention, counselling, and advocacy skills
· Knowledge of local housing markets, homelessness system practices, and community resources
· Excellent communication, documentation, and organizational skills
· Ability to work independently, exercise sound judgment, and manage a complex caseload
· Comfort working with individuals experiencing mental health, addictions, and other barriers
· Ability to maintain professionalism, confidentiality, and strong boundaries
· Experience with HIFIS is an asset
Qualifications Required
· Community College Diploma in a social services field (or equivalent education/experience)
· Minimum two years of experience in housing-related case management or homelessness prevention is preferred
· Knowledge of Housing First, Residential Tenancies Act, and Coordinated Access systems
· Proficiency with computer applications and database entry
· Valid Driver’s Licence, reliable personal vehicle, and appropriate insurance (required for client transportation)
Working Conditions
· Frequent work in the community and single-staff situations
· Regular interaction with clients who may be in crisis or present as agitated
· Transporting clients using your personal vehicle when necessary
· Regular entry into client homes; discretion and professionalism required
· High-pace environment with urgent client needs and frequent interruptions
· Pressure to meet immediate housing demands and maintain caseload stability
