FULL-TIME PERMANENT POSITION
Youth Habilitation Quinte Inc. requires a Transitional Connector who will promote independence and quality of life through the coordination of required support services for young people 16 to 24 years of age. This contract is to commence in July. The office will be based in Smiths Falls and the position will service Lanark, Leeds & Grenville.
The successful candidate will oversee the transition of the young person from the children’s mental health system to the adult mental health system and ensure the appropriate supports are in place to facilitate the completion of the transition process.
The candidate must possess excellent communication, organization and facilitation skills, have individual, group and family mental health assessment and counselling skills and have a strong working knowledge of the children’s and adult mental health systems in Lanark, Leeds & Grenville.
The candidate will possess an undergraduate degree in a related field with a minimum of five years of experience. A valid driver’s licence and a dependable vehicle are required.
